Please send a headshot of yourself with a neutral background and even lighting. Your photo will be printed on the front of the card, along with the ADA guidelines on discrimination. The QR code in the front will link you to Ada guidelines on mask exemptions. The cards will come with the text as shown in the pictures of the front and back. The Cards will include custom Medical Exemption holographic stickers along with a generic holographic laminate, a card protector and a custom Red (MASK EXEMPT) red lanyard. You can also choose Black (Medical Exemption) and a Red (Medical Exemption). Please specify what you want through eBay contacts.
The title " The United States of America" is to state in what geographic location the ADA guidelines are originated from. In summary, these cards simply state ADA guidelines, what is on CDC website with your picture on it. This is not a government or corporate ID card, nor made to be one. There are no governmental emblems or Insignias. It is just a beautiful piece of art.
